Whenever possible prescription refills should be done electronically through your pharmacy. You can call your pharmacy and ask for an electronic prescription refill and this will be done online. If your pharmacy does not have electronic capability you may call our medical assistant with your request during office hours.How do I get a refill on my prescription?

Payment is expected at the time services are rendered unless other arrangements have been made. Please have your applicable co-pay payment ready when services are rendered.

Please be sure to bring your insurance card, referral if required by your insurer and Worker’s Comp authorization as well as any additional paperwork. We also do require patient forms which are available for download and you can fill these out prior to your visit.

In addition we participate in all Aetna insurance plans, all Blue Cross/Blue Shield plans, Connecticare, Cigna, United Health Care, Husky plans for 21 and under and all private health insurance plans unless otherwise excluded.
